Drug Supply Chain Security Act (DSCSA) Notice
For pharmaceutical orders, a valid Global Location Number (GLN) is required to comply with the Federal Drug Supply Chain Security Act (DSCSA). Concordance Healthcare Solutions LLC ("Concordance") reserves the right to delay or hold shipment of pharmaceutical products until the Customer provides the required GLN.
California Proposition 65 Notice & Customer Responsibilities
Concordance Healthcare Solutions LLC ("Concordance") is a distributor of healthcare products and does not manufacture, design, or formulate the products it supplies. Information regarding product composition, including the presence of chemicals subject to regulatory disclosure requirements, is provided by the applicable manufacturer or as otherwise required by law.
In compliance with California Proposition 65, formally known as the Safe Drinking Water and Toxic Enforcement Act of 1986, Concordance provides the following notice for products shipped into the State of California:
WARNING: This product may expose you to chemicals known to the State of California to cause cancer and/or birth defects or other reproductive harm.
Customers and end users who purchase or use products distributed or shipped by Concordance into the State of California are responsible for determining and complying with all applicable California Proposition 65 requirements, as well as any other applicable federal, state, or local laws and regulations.
Except as expressly stated in writing, Concordance makes no representations regarding whether any product contains chemicals subject to Proposition 65. To the extent permitted by law, Concordance disclaims liability and shall not be responsible for any claims, damages, losses, penalties, or liabilities arising out of or related to a customer’s or end user’s failure to comply with Proposition 65 or other applicable legal requirements.
New York Chemical Disclosure & Consumer Notice
Certain products distributed by Concordance Healthcare Solutions LLC ("Concordance) may contain chemicals or substances that are regulated under applicable New York State laws and regulations, including those relating to consumer protection, environmental safety, and occupational health.
Concordance is a distributor of the products it supplies and is not the manufacturer, and therefore does not control the design, formulation, or production of such products. Product information and regulatory disclosures are provided based on information supplied by the applicable manufacturer or as otherwise required by law.
Healthcare professional, customers, and end users are responsible for independently reviewing all relevant product documentation, including Safety Data Sheets (SDS), product labels, package inserts, instructions for use, and other manufacturer‑provided materials, to determine whether a product is appropriate for its intended use and compliant with applicable federal, state, and local laws and regulations.
This notice is provided for purposes of transparency and regulatory awareness only and is not intended to constitute a warning of misuse, hazard, or exposure under normal conditions of use, nor does it replace or supersede any legally required product labeling, warnings, or disclosures. For additional information regarding product composition, regulatory status, or New York‑specific compliance obligations, customers should contact their Concordance representative or the product manufacturer directly.
PFAS (Per- and Polyfluoroalkyl Substances) Transparency Notice
Concordance Healthcare Solutions LLC ("Concordance") recognizes the increasing attention surrounding Per‑ and Polyfluoroalkyl Substances (PFAS), a group of synthetic chemicals that may persist in the environment and, in some cases, the human body.
Concordance distributes a wide range of healthcare products sourced from third‑party manufacturers. As a distributor, Concordance does not design, manufacture, or control the formulation of the products it supplies. Product composition information, including any disclosures related to PFAS, is provided by the applicable manufacturer or as otherwise required by law. Concordance does not make independent representations regarding the presence or absence of PFAS in products unless such information is expressly stated in writing.
Customers and end users are encouraged to review all available product documentation, safety data sheets, labels, and manufacturer‑provided materials, and to assess product suitability in light of applicable federal, state, and local requirements related to PFAS. Customers remain responsible for determining compliance with any laws or regulations governing product use, restrictions, or reporting obligations.
This notice is provided to promote transparency and regulatory awareness and does not replace, supersede, or modify any manufacturer disclosures or legally mandated warnings.
3TG (Tin, Tantalum, Tungsten, and Gold) Disclosure
Concordance Healthcare Solutions LLC ("Concordance") is a distributor of the products it supplies and is not the manufacturer, and therefore does not control the design, formulation, assembly, or production of such products.
While Concordance is not a publicly traded manufacturing company and is not directly subject to the conflict minerals reporting requirements of the Dodd‑Frank Wall Street Reform and Consumer Protection Act, certain manufacturers whose products are distributed by Concordance may be subject to those requirements. To support responsible sourcing initiatives, Concordance may, upon request and to the extent commercially reasonable, assist manufacturers by identifying, based on information available to Concordance, whether 3TG minerals (tin, tantalum, tungsten, and gold) contained in supplied products may have originated from the Democratic Republic of the Congo or adjoining countries.
Where applicable, Concordance’s efforts are intended to be consistent with the Organisation for Economic Co‑operation and Development (OECD) Due Diligence Guidance for Responsible Supply Chains of Minerals from Conflict‑Affected and High‑Risk Areas. Information regarding the origin of 3TG minerals is provided based on manufacturer‑supplied data and is subject to the limitations of upstream supply chain transparency.
Customers are responsible for determining their own compliance obligations under applicable laws and regulations relating to conflict minerals. To the extent permitted by law, customers agree to indemnify and hold harmless Concordance from any claims, liabilities, penalties, or damages arising out of or related to a customer’s failure to comply with such obligations.
Patient Privacy & HIPPA (Health Insurance Portability and Accountability Act) Awareness
At Concordance Healthcare Solutions LLC ("Concordance"), protecting patient privacy and handling information responsibly are core to how we operate.
Concordance does not request or require access to individually identifiable patient health information in order to provide distribution or supply services, and we do not serve as a “business associate” under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
In certain situations, such as when Concordance team members are present in healthcare facilities, there may be incidental exposure to clinical environments. In these instances, Concordance personnel are trained to respect patient confidentiality and handle any incidental information in a manner consistent with HIPAA and other applicable privacy laws. Our customers and healthcare partners remain responsible for managing patient information within their own systems and operations.
